United Way's Liability Agreement
By signing this form, I give my permission for myself, and any minors listed to take part in activities at the United Way Community Center. I understand that we may interact with United Way staff and volunteers, and that participation may involve some unforeseen risks. I agree on behalf of all participants listed with the statements below.
1. I understand that I am responsible for any children I bring to the Center. United Way staff and volunteers are not responsible for their well-being. It is my responsibility to make sure the children I bring stay at the Center, are not disruptive, and are participating in their activities. Children may not be at the Center unless they are registered for and attending an activity, with a parent present. Staff and volunteers are not able to supervise my children while I or they are attending an activity. The only exception is volunteers assigned specifically to childcare, but even then, they are not liable for my children. Childcare is only available for children ages 9 and under, unless otherwise stated. I understand that my children or I may be asked to leave if we do not follow these rules.
2. I understand that I cannot hold United Way Community Center, volunteers, and/or other participants responsible for any unforseen risks/damage that may take place during the participation of any activity.
3. I understand that I, as the parent/guardian/responsible party of the participants named in this form, will assume responsibility for any damage that my children or I might cause at the United Way Community Center.
4. I understand that some of the activities at the United Way are only appropriate for adults. Due to this, I, as the adult, will only let the underage applicants participate in activities that are age appropriate. I also understand that if any named minor wants to attend an activity that is not age appropriate, an adult must accompany him/her.
5. I understand that when United Way Community Center staff, volunteers, or participants have "reason to believe that a child has been subjected to abuse or neglect" or "observes a child being subjected to conditions or circumstances which would reasonably result in abuse or neglect" then that person is required by law to "immediately notify the nearest peace officer, law enforcement agency, or office of the division" (Utah Code; Title 62A, Chapter 4a, Part 4, Section 403).
